For EQ, call or write for it. Don't try to pull it online. A lotta folks report it don't work, and I myself can attest to the online request for EQ not working.

When I fill out paperwork at a doc's  office, I conveniently "forget" to fill in my SSN.  (If it's an emergency, I claim amnesia as part of my symptoms.)

You are not required to provide it to a medical provider.  Yes, I know, someone can pull a report without the SSN.
